Maryland's Total Cost of Care Model
Maryland’s Total Cost of Care (TCOC) model is an expansion of the All-Payer model initiated in 2014. The All-Payer model created global budgets for Maryland hospitals to reduce Medicare spending and improve quality of care. This strategy disincentivizes unnecessary hospitalizations and incentivizes patient-centered care by providing hospitals with a fixed amount of revenue for the upcoming year instead of paying for care on a fee-for-service basis. The All-Payer model produced successful outcomes in reducing Medicare spending and improving patient care quality. The TCOC model extends the successes of the All-Payer model to primary care providers and creates more incentives for providers to work collectively to promote patient-centered care and health equity.
