Race, Equity, and Social Determinants of Health

Access to stable housing, food quality, social support networks, and other social factors are critical in shaping health outcomes. These factors are known as social determinants of health and they are rooted in unjust systems. This year, Maryland Philanthropy Network’s Health Funders will be conducting a series of programs exploring Social Determinants of Health (SDoH) topics, to better understand the roles of all funder groups in advancing health equity.

Please join the Health Funders for a presentation to kick off the SDoH series. This presentation will include a grounding in health equity and racial justice, and updates from The Centers for Medicare and Medicaid Services’ Accountable Health Communities Program in Maryland. The program will also feature a screening of "Inherit the Earth", A mini-documentary produced by Saint Agnes Healthcare about SDoH issues in South Baltimore.

  • Shelly Choo, MD, MPH Chief Medical Officer, The Baltimore City Health Department 
  • Olivia Farrow, J.D. Director of Community Engagement, Saint Agnes Healthcare
