Advance Care Planning: Mobilizing the Community
Join the Affinity Group on Aging as we present another program in our Innovative Aging series. Tiffany Callender, Program Director, the Horizon Foundation will speak on the Foundation’s groundbreaking initiative Speak(easy) Howard.
Speak(easy) Howard is a public education/engagement campaign that encourages County residents to identify a health care agent and to think about their preferences for health care. Speak(easy), launched as a pilot in 2016, has already reached thousands of Howard County residents.
Tiffany Callender manages several of the Horizon Foundation’s initiatives that focus on Healthy Kids and Families and Healthy Aging. These include Speak(easy) Howard as well as the Foundation’s mental health programming. Ms. Callender’s background includes clinical experience and a strong foundation in strategic planning. She holds a MSW from Temple University and a BA in psychology from Eastern University.
Following the program, members of the Affinity Group on Aging and our community partners will hold a brief planning meeting for our programs during 2019.
